In order to complete your application, you will need the following:
- The Nike store closest to you (note: organizations over 30 miles from the nearest NikeRetail location will be paired with Nikestore.com.)
- Contact information, website, and mission statement
- The 501(c)(3) number of your organization
- The number of kids, volunteers/ participants involved
- Current program funding details and partners
- Plan for sustaining program
- Project impact and scope
- How Nike will influence your community

What is the judging criteria?
- Applicants must adhere to the Nike giving guidelines
- Applicants must provide programming that encourages young people to create a better life through sports
- Applicants who qualify for the Community Store Grant Program in Portland, Oregon, South Chicago, Illinois and New Orleans, Louisiana may also provide mentoring and/or economic development opportunities.
- Applicants must be 501 (c) 3 organizations as classified by the Internal Revenue Service.
